Care Communities
One of our key priorities within St Helens Cares is focussed on the development and establishment of 4 Care Communities across North St Helens; Central St Helens; Newton & Haydock; and South St Helens.
Care Communities are a way of working together in a truly multi-disciplinary way, to deliver the right personalised care / support for a whole person approach and ensuring a seamless journey through healthcare and social services in St Helens Borough. Within our 4 Care Communities, we aim to build on our existing local teams, develop strong professional relationships and create new working relationships to provide health and care services for our population in response to increasing demand and a changing workforce to achieve the following key objectives:

Professional Groups of people working effectively together for the benefits of patients.

Professional Groups of People working proactively as well as reactively.

People and patients able to access relevant information in a timely manner.

Holistically produced useful care plans.
The above will enable us to:

Connect and utilise the whole of healthcare and social support services in St Helens Borough, including the voluntary sector.

Ensure processes are in place to enable Care Community partners from different organisations to work effectively together for the benefit of the patient.

Establish procedures within the Care Community that ensures holistic, patient centred care can be given to each person seamlessly.

Embed processes to support people with complex health needs.

Build focus on early intervention and prevention by empowering, enabling and supporting people to “self-care first”.
